Responsibilities

  • Develop working relationships with internal sales team and facility staff to create excellent admission outcomes.
  • Handles phone inquiries, checks for referrals, and processes them expediently and accordingly.
  • Communicates with the Liaison and referral source, notifying them it has been received and an answer will be provided promptly on acceptance or denial of the referral.
  • Reviews health records prior to accepting referral.
  • Creates new patient digital charts.
  • Verifies primary insurance coverage and obtains authorization by calling insurance provider.
  • Costs out medications and obtains approval from the Administrator; once approved, notifies Liaison to complete referral evaluation.

Qualifications

  • Minimum of one (1) year experience with admissions in a skilled nursing facility, hospital discharge, or case management.
  • High School graduate / GED required.
  • Type 40 words per minute.
  • Knowledgeable of medical practices, terminology and procedures, as well as laws, regulations and guidelines that pertain to home health.
  • Able to relate positively, favorably and cooperatively with others, including physicians, associates, and personnel of outside agencies and organizations.
  • Must be capable of working under pressure in a busy setting, projecting a friendly and professional image. Must be versatile and adapt to changing job duties and priorities, and/or emergent or stressful situations.
